The Day the Cast Comes Off
It's the day you've been waiting for. The cast is removed, and you expect to immediately move your arm or leg like before. But instead, it looks thinner, feels stiff, and barely moves. This is completely normal — and this is exactly when the real recovery begins.
Why Your Limb Looks and Feels Different
Muscle Atrophy
When a limb is immobilized in a cast for weeks, the muscles shrink. You can visibly see the size difference compared to your other limb. This is called disuse atrophy, and it's expected.
Joint Stiffness
Without movement, joints become stiff. The capsule around the joint tightens, and synovial fluid (the joint's natural lubricant) decreases. This makes movement painful and limited.
Reduced Bone Density
Immobilization also reduces bone density in the affected area, making it temporarily more fragile. This is why a gradual return to activity is essential.
The Rehab Roadmap
Phase 1: Gentle Range of Motion (Week 1–2)
Your physiotherapist will guide you through gentle movements to break through stiffness. These are slow, controlled movements within your comfort zone. Never force a stiff joint.
Phase 2: Strengthening (Week 3–6)
Once mobility improves, we introduce light resistance exercises. Isometric exercises — contracting muscles without moving the joint. Light resistance bands. Gradually increasing load as the muscles respond.
Phase 3: Functional Training (Week 6+)
This is where we train your limb for real-life activities. Gripping and carrying objects (for arm fractures). Walking on uneven surfaces (for leg fractures). Balance and proprioception exercises. Sport-specific training if applicable.
Common Mistakes After Cast Removal
**Skipping physiotherapy** — "It will heal on its own" is the most common and most harmful assumption. **Doing too much too soon** — Overzealous exercise can re-injure the healing bone. **Ignoring persistent swelling** — Some swelling is normal, but if it doesn't reduce, consult your doctor.
The cast protected your bone while it healed. Now physiotherapy will rebuild everything around it.
